Nervous system dysregulation – how to recognise it?
Physical symptoms
When the nervous system is in threat mode for too long, the body begins to “speak” first.
Typical symptoms that in clinical practice are often associated with chronic stress:
chronic muscle tension, especially in the neck, shoulders and jaw,
tension-type headaches, a sensation of a “band” around the head,
tightness, pressure or a “block” in the chest,
digestive problems (bloating, diarrhoea, constipation that worsen with stress),
heart palpitations, a feeling of “strong beating” with a relatively small trigger.
Emotional and cognitive symptoms
Nervous system dysregulation affects how you experience emotions and how your thinking works.
Commonly appearing:
difficulties falling asleep and waking up at night,
racing thoughts, analysing “what if…” scenarios,
panic attacks, a sense of “anxiety without reason”,
irritability, explosiveness, low frustration tolerance,
difficulty “switching off” even in objectively safe conditions.
Behaviour
Behaviour often becomes an attempt at self-regulation – more or less effective.
You may notice in yourself:
overload from stimuli (noise, emails, social media) and a need to “disconnect” after work,
frequent reaching for “calmers”: phone, food, alcohol, working late,
avoiding contact with people, isolating, cancelling meetings,
or conversely – overactivity, difficulty being alone with yourself.
5 pillars of practical nervous system regulation
When working with patients I usually structure a regulation plan around five pillars. These are not “magic techniques”, but concrete habits that support the physiology of the nervous system.
Breath as the first regulator
Breath is the quickest conduit between the conscious “self” and the autonomic system.
Slower, deeper diaphragmatic breathing increases parasympathetic activity (vagus nerve), which helps lower heart rate and tension.
Short breathing practices do not by themselves cure depression or anxiety disorders, but they help to exit an acute alarm state.
Examples:
Box breathing (4–4–4–4): inhale through the nose for 4, hold for 4, exhale for 4, hold for 4; repeat for 1–3 minutes.
Extended exhale (4–6 or 4–8): inhale for 4, gentle exhale for 6–8; do not force, preferably “soft” rather than forceful.
Instructions: sit comfortably, place a hand on your belly, allow it to rise gently on the inhale and fall on the exhale. If thoughts drift away – return to counting breaths.
Movement and “shaking out” tension
A small, regular physical activity improves autonomic regulation, sleep and mood.
It’s not only about workouts at the gym, but about a signal to the nervous system: “the threat has passed, energy can be discharged”.
Helpful activities:
gentle walks, rhythmic cycling, dancing,
gentle “shaking out” of the arms and legs, shaking the shoulders, rocking the body,
stretching exercises after a day at the computer.
The point is not an intense “cutting-edge workout”, which can itself be another stressor for an overloaded body, but gentle movement that the body associates with safety.
Grounding and body contact
When the nervous system is on alert, attention easily “detaches” from the body and chases thoughts. Grounding helps return to the here and now.
Example practices:
focus on the feet: feel the weight of the feet on the floor, distribution of pressure, temperature;
mindful sitting: notice the weight of the body on the chair, contact points, the back touching the backrest;
a simple body scan – from the top of the head to the toes, without judging, just noticing tension.
Exercise “5–4–3–2–1”:
name 5 things you can see,
4 things you can hear,
3 things you can feel with your body,
2 things you can smell,
1 thing whose taste you remember.
This redirects attention away from internal noise to specific stimuli.
Mindfulness, relaxation, meditation
Research on mindfulness programmes (e.g. MBSR) shows that regular mindfulness practice can reduce perceived stress levels and symptoms of anxiety and depression to a small to moderate degree.
Well-designed relaxation techniques (e.g. autogenic training, Jacobson’s progressive muscle relaxation, yoga nidra) lower physiological arousal and support self-regulation capacity.
In practice:
short daily mindfulness practices (e.g. 5–10 minutes focused on the breath),
gentle yoga aimed at calming,
yoga nidra sessions before sleep,
can be a valuable supplement to psychotherapy and pharmacotherapy, but do not replace treatment for more severe disorders.
Lifestyle: sleep, light, diet
Sleep, exposure to daylight and eating habits strongly influence autonomic activity and stress resilience.
Key elements:
regular sleep times, limiting screens and bright light in the evening,
morning exposure to natural light as a signal that “the day has started”,
contact with nature, which in studies is associated with reduced stress levels and improved mood,
regular meals, attention to dietary variety for the gut microbiota (the gut–brain axis).
Increasing evidence indicates that the state of the gut microbiota may influence mood, anxiety and stress response, although this is not yet a simple “diet recipe for stress”.
How to build a personal regulation plan – step by step
The goal is not to create a perfect day, but to introduce a few realistic, repeatable anchor points for the nervous system.
Morning
5 minutes of calm breathing (e.g. inhale 4, exhale 6) before reaching for the phone.
A moment of movement: short stretching, a few bends, a walk with the dog.
Exposure to daylight – open the blinds, step onto the balcony, a short walk instead of scrolling.
During the day
micro-breaks every 60–90 minutes: 1–2 minutes of breathing, get up from the desk, loosen the shoulders.
consciously limiting stimuli: muting notifications, checking email at set times.
take a short walk between meetings instead of “pushing through” with another coffee.
Evening
a “calming ritual” 30–60 minutes before bed: put away screens, dim the lights, gentle music.
gentle stretching/yoga, a warm shower or bath, a relaxation technique (e.g. yoga nidra, muscle relaxation).
note down a few things from the day (so you don’t “chew” them over in your head in bed).
When self-regulation is not enough?
Nervous system regulation techniques can significantly improve quality of life, but they do not replace professional help in situations when:
anxiety or depressive symptoms are severe and persist for weeks,
thoughts of giving up or suicide appear,
you have chronic sleep disorders (insomnia, frequent awakenings) despite attempts at sleep hygiene,
eating disorders or addictions occur (alcohol, medications, substances, internet),
somatic symptoms are sudden and severe: intense chest pain, shortness of breath, sudden neurological symptoms – in that case urgent medical consultation is required.
In such situations regulation techniques can support therapy and pharmacotherapy, but will not replace them. The role of the physician is to make a diagnosis, propose treatment in line with guidelines and only then to choose self-regulation tools tailored to your life.
