If you’re asking these questions, it’s most likely that:

  • you’ve been running on reserve for a long time,

  • work has become too burdensome for your mental health,

  • and at the same time scheduling an in‑person visit feels like one more hill you don’t have the strength to climb.

In this text I won’t promise “L4 in 15 minutes”.
Instead:

  • I’ll explain when an online psychiatrist can issue an L4 and on what basis,

  • I’ll show how medical leave from a psychiatrist works in practice,

  • I’ll tell you what you can do on your side so that the consultation is genuinely about you, not about “getting the paperwork”.

Always with one sentence in the back of my mind:
“I see the person as a whole — and I guide them that way.”


Can an online psychiatrist issue an L4?

Short answer: yes.

In the Polish system a e‑medical leave (e‑ZLA) can be issued by a doctor — including a psychiatrist — both during an in‑person visit and during an online consultation (teleconsultation, video visit).

This means that:

  • an online psychiatrist can issue an L4 if during the visit they determine that your mental state prevents you from working,

  • such a leave is legally equivalent to one from an in‑person office — it is sent electronically to ZUS and your employer, and you can view it on IKP or PUE ZUS.

Importantly:

  • this is NOT a service for “L4 on request”,

  • a doctor must always assess your health based on an interview, and often also on documentation, previous treatment, etc.

The law is clear: a medical leave may be issued only by a physician who has the authorization from ZUS and sees medical grounds for it.


When is a sick leave from an online psychiatrist medically justified?

Let’s put this in plain terms.
From my point of view, an L4 from a psychiatrist — including online — makes sense when:

  • your mental state genuinely prevents you from doing your job:– you can’t concentrate,
    – you make mistakes that could have serious consequences,
    – panic attacks, severe insomnia, or suicidal thoughts make it impossible to function in an “8 hours a day” rhythm.

  • symptoms last longer than a few bad days,

  • symptoms are not just “tiredness after one project”, but the result of accumulating stress, burnout, depression, anxiety disorders, etc.

Examples of situations in which an online psychiatrist may consider L4:

  • an episode of depression with marked low energy, sleep disturbances, and difficulty fulfilling duties,

  • severe anxiety disorders, panic attacks related to work,

  • occupational burnout evolving into a mood disorder,

  • exacerbation of a chronic psychiatric condition (e.g. bipolar disorder, anxiety disorders) where continuing work is currently too burdensome.

There is no medical justification for L4 if:

  • “I have a bad day and don’t feel like going to work”,

  • “I’m having issues with my boss, but otherwise I function normally”,

  • “I want a longer weekend”.

I know this sounds strict, but L4 from a psychiatrist — including online — is part of treatment, not a way to take a break from an unpleasant job.


What does an “online psychiatrist L4” visit look like in practice?

Technically it can be very simple: you book a time, get a link or phone number, and connect at the scheduled hour.
Substantively — it is a full medical visit, not a form to “click L4”.

1. You book a consultation

  • you choose a time with a specific psychiatrist online,

  • you fill out a short preliminary questionnaire (symptoms, medications, illnesses, work situation),

  • sometimes you indicate if you have referrals or documentation.


2. Conversation with the doctor – interview about your functioning

During the consultation the psychiatrist:

  • asks the reason for contact,

  • checks how long symptoms have lasted,

  • assesses how they affect your work, relationships, self‑care,

  • evaluates mood, anxiety, sleep, appetite, energy level, suicidal thoughts.

This is the moment when it’s important to speak honestly – not “for L4”, but about how things really are. As psychiatrists emphasize, the best way to get appropriate help is to tell the truth, not what seems like the right thing to say.


3. Decision about L4 (or not)

Based on the interview the doctor decides:

  • whether treatment is needed (e.g. medications, therapy),

  • whether medical leave is indicated, and if so – for what period.

If they determine that an online psychiatrist L4 is medically justified, they issue an e‑ZLA:

  • it is sent directly to the ZUS system and to the employer,

  • you can check it on IKP / PUE ZUS,

  • as a rule, a leave can be issued up to 3 days retroactively.

If they do not see indications for L4 – they may propose other forms of help:

  • treatment,

  • a follow‑up appointment,

  • referral to a psychotherapist,

  • recommendations regarding lifestyle, work, rest.


What does the employer see on an L4 from an online psychiatrist?

This is a common and important safety question.

The employer:

  • sees the fact that an L4 was issued,

  • sees the period of the leave,

  • does not see the diagnosis or health details,

  • does not have information that it is an “L4 from an online psychiatrist” – the reason for incapacity is visible only to the doctor and ZUS.

This means you do not have to disclose to your boss details like “I have depression, I saw an online psychiatrist”.
You have the right to say as much as you feel safe sharing.


How long does an L4 from an online psychiatrist last?

There is no single magic duration.

In practice:

  • the length of leave depends on diagnosis, symptom severity and type of work,

  • it can be a few days, a few weeks, or longer – if the mental state truly prevents work,

  • when extending leave the doctor assesses treatment progress and whether return to work is realistic.

Financial rules (80% salary, 100% in certain situations, day limits) are the same as for any other medical leave – regardless of whether it was issued by an online psychiatrist or a general practitioner.


How to prepare for an “online psychiatrist L4” visit so it’s about you, not paperwork?

Things you can do for yourself (not for the system):

  • Write down your symptoms:
    – since when they persist,
    – how much they interfere with work (specific situations),
    – how they affect sleep, appetite, daily duties.

  • Think about what you’d like to say about your job:
    – what is most burdensome,
    – what resources and support you have,
    – whether you’ve already tried something (vacation, job scope change, therapy).

  • Allow yourself to say:

    “I’m wondering whether in my situation a sick leave is necessary. I would like the doctor to assess whether there is a medical basis for it.”

It’s completely reasonable to ask this.
The doctor’s role is to decide – your role is to come with the truth about your condition, not a rehearsed script “for L4”.

Can an online psychiatrist issue an L4?

Yes. An online psychiatrist can issue an electronic medical leave (e‑ZLA) if during the consultation they determine that your mental health prevents you from working. Such an L4 has the same validity as one issued in a clinic.

Yes. An L4 from an online psychiatrist is a standard e‑ZLA – it is sent electronically to ZUS and the employer. The employer sees only the fact of leave and its period; they do not see the diagnosis or that it was issued by a psychiatrist.

The most important thing is to tell the truth about how you feel and how symptoms affect your work. You can directly say you are considering L4 and ask the doctor to assess whether there are medical indications. The doctor will decide based on the interview, not a prepared “script”.
